Appointments are available within days, not months. Visits run as long as they need to. And unlike every other concierge practice, there is no monthly membership fee, no annual contract, and no charge simply for being a patient here. You pay for what you use — nothing more. In exchange, you get something increasingly rare in modern medicine: a physician with time, attention, and a genuine investment in your long-term health.

How this Model Works
Primary physician access — without the traditional strings attached.
At Denali East Group, Dr. Patel can serve as your primary physician under a concierge model that is fundamentally different from what most people think of when they hear "primary care." There is no panel of thousands of patients, no 10-minute appointment slots, and no front-desk staff who barely know your name.
This is also not a traditional concierge model — meaning you will never pay a monthly retainer or annual membership fee simply to have access to the practice. You pay when you are seen. When you need Dr. Patel, you book. When you don't, you owe nothing.
What you get in exchange for the pay-per-visit structure is something most patients have never experienced: a physician with actual time for you, a relationship built over visits, and access within days when something comes up.

The Denali East Group Difference
Every other concierge practice charges a membership fee.
We don't.
Traditional concierge medicine typically requires patients to pay $100–$400 per month — or $1,800–$5,000 per year — just to be a patient, before a single visit takes place. You pay whether you are seen once or twenty times.
At Denali East Group, there is no such fee. You are never charged for the privilege of being on our books. Our model is simple: pay per visit, pay per service, pay per ‘touch’, pay for what you actually use. Nothing more.
This means a patient who sees Dr. Patel three times a year pays for three visits. A patient who sees him once pays for one. And a patient who goes a full year without needing care pays nothing at all — and can walk back in the door whenever they are ready.

Dr. Patel welcomes pediatric patients under the same concierge primary care model. For families who want a physician-led, relationship-based approach to their child's health — one that respects parental involvement and informed decision-making — this model is a natural fit.
Dr. Patel's clinical background includes extensive experience with children across a wide range of acute and chronic conditions, from infancy through adolescence. His approach is family-centered, communicative, and free of the judgment or pressure that some families encounter in traditional pediatric settings.
Vaccine-hesitant or non-traditional vaccine schedule families are welcomed without shame. Medical vaccine exemption forms are available when clinically appropriate following a proper evaluation.

Understanding the pay-per-touch model
Denali East Group operates as a pay-per-touch concierge practice. Every clinical service, physician interaction, and medical encounter carries an associated fee — and that is precisely what makes this model work. It funds the small patient panel, the unhurried appointments, and the direct physician access that traditional practices cannot offer.
Secure patient portal messaging is available at no charge for routine, non-clinical questions. However, telehealth visits, office visits, phone consultations with Dr. Patel, medication management, and all other physician encounters are subject to service fees. There are no hidden charges — current pricing is provided at the time of scheduling or before services are rendered.
We are a self-pay practice and do not accept insurance however superbills are available upon request and can be submitted to your insurance company for potential reimbursement. We cannot guarantee what your insurer will reimburse — we recommend contacting them in advance.

No — and this is one of the most important ways Denali East Group differs from traditional concierge medicine. Most concierge practices charge $150–$400 per month simply to be a patient. We do not. You are never charged for being on our books. You pay when you are seen and when services are rendered — nothing more.

It means that every clinical service, physician encounter, procedure, and interaction carries an associated fee. Office visits, telehealth visits, scheduled phone calls with Dr. Patel, medications dispensed through the practice, lab interpretations, and documentation requests are all individually priced and transparently disclosed before services are rendered. The one exception is routine patient portal messaging for non-clinical questions, which is complimentary. Current pricing is provided at the time of scheduling.

Our model is best described as primary physician access rather than a traditional PCP panel relationship. Dr. Patel can serve as your ongoing physician and manage your care over time, but he does not carry the full administrative infrastructure of a traditional PCP office — including coordination with insurance networks, hospital-based care coordination, or the full range of forms and documentation that a traditional PCP manages. We recommend being upfront with your insurer about this distinction.

No. At Denali East Group, we do not call in prescriptions based on a phone description of symptoms or a patient portal message. Every treatment decision begins with a proper evaluation — because that is what good medicine requires. If you need a prescription, you need a visit. This standard of care protects you, and it is one we do not compromise on.

We do not bill insurance directly. However, we provide superbills upon request, which you can submit to your insurance company for potential reimbursement. We recommend contacting your insurer in advance to understand your out-of-network benefits. Lab work ordered through LabCorp may be billed to your insurance separately.

Appointments are typically available within days, not months. This is one of the core commitments of the concierge model — you will not be told the next available appointment is in three months. If the online schedule does not show availability that works for you, reach out directly and we will do our best to accommodate you promptly.

Yes. Chronic condition management is a core part of what Dr. Patel offers as a primary physician. This includes hypertension, diabetes, thyroid disorders, metabolic syndrome, and related conditions. He tracks your labs and history over time and adjusts your care plan accordingly. Complex multi-specialty cases may also require specialist involvement, which Dr. Patel can facilitate through referrals.

Yes, and there are two important categories to be aware of before establishing care with Dr. Patel.
First, certain medications are dispensed directly through our practice and will not be sent to an outside pharmacy. This includes testosterone, estrogen, progesterone, peptide therapies, sildenafil, tadalafil, clomiphene, and other specialty or compounded medications used in our hormone optimization and wellness programs. If you receive one of these therapies through Denali East Group, you purchase it directly from us — period. This is how we ensure consistent quality, accurate dosing, proper handling, and uninterrupted supply. Patients who prefer to fill these medications at their own pharmacy will find that a traditional primary care office is better suited to that preference.
Second, we do not prescribe controlled substances. This includes opioids, stimulants, and benzodiazepines — including benzodiazepines used in the management of anxiety, panic disorder, or other psychiatric conditions. Patients who require controlled substance prescriptions or ongoing psychiatric medication management should establish care with a provider equipped to appropriately manage those therapies.
For all other medications, Dr. Patel is happy to e-prescribe to your pharmacy of choice when clinically appropriate — meaning following a proper evaluation, not based on a phone call or portal message description of symptoms.

Dr. Patel can provide specialist referrals as part of your care. Keep in mind that our practice does not coordinate directly with insurance networks the way a traditional PCP office does, so insurance-driven referral requirements may vary depending on your plan. We recommend understanding your coverage before scheduling with a specialist.

If we are in a position to see you outside normal availability, we are happy to do so. Reach out via text to (252) 368-0090 if you do not see available openings on the booking site schedule. Please note that after-hours visits carry an additional after-hours fee, and while we do our best to respond promptly, we cannot guarantee availability outside scheduled hours.

Routine work and school excuse notes are included as part of the visit fee. Other medical documentation, letters, and form completion requests are subject to a document completion fee. Our practice does not complete disability paperwork or FMLA documentation — a traditional PCP office is better equipped to handle those.

Urgent care clinics handle walk-in acute needs with whoever happens to be working that day — a different provider every time, no continuity, often long waits. At Denali East Group, you always see Dr. Patel directly, appointments are scheduled (not walk-in), and the relationship is ongoing. Beyond acute care, Dr. Patel also manages your long-term health, monitors your labs over time, and provides the kind of continuity that urgent care simply cannot offer.

Book your first visit directly through our online scheduling system. No referral is needed, and there is no enrollment process or membership to sign up for. Simply book the In-Person Clinic Visit option, complete your intake paperwork, and come in. Dr. Patel will take it from there.
